EYA Employee Benefits
We are pleased to provide you with a summary of Eckerd Youth Alternatives' (EYA) BeneFlex benefit program. EYA recognizes that benefits are an important part of a total compensation package and, because of this, we give our employees the opportunity to make selections that best meet their benefit needs. We are striving to provide benefit choices that allow employees to custom tailor their benefits.
Eligibility
Regular full-time employees who work at least 30 hours per week are eligible for both mandatory and benefit choice coverage. Spouses and dependent children of eligible full-time employees are also eligible for health, dependent life insurance, dental, vision, cancer, and specific disease benefit plans.
All benefit coverages are in effect for both employees and their dependents (if coverage was elected) effective the 61st day of employment. The only exception is the Prepaid Dental Plan Option (available to Florida employees only) for which the effective date of coverage is determined when the application is received (a maximum of a 60-day waiting period).

Pretax Advantage
An added advantage of the BeneFlex program is that premium payments you contribute for both health insurance and dental insurance are automatically paid on a pretax basis. When you pay for your benefits with pretax dollars, you are actually reducing your taxable income.

Mandatory Coverage
Mandatory coverage are considered your basic level of insurance protection.
The BeneFlex mandatory coverage provided at no cost are:
- Basic Life Insurance - One times your base annual earnings increased to the next 10,000 paid by EYA
- Accidental Death and Dismemberment Insurance (AD&D) - One times your base annual earnings increased to the next 10,000 paid by EYA
- Short-term Disability Insurance - 60% of your base weekly earnings to a max of $1,900 per week paid by EYA
- Business Travel Accident Insurance - up to $100,000 paid by EYA
The Beneflex mandatory coverage that requires an employee contribution is:
- Enhanced Long-term Disability Insurance - 60% of base monthly earnings to a max of $8,000 per month paid by employee

Health Insurance Plan Options
You will be eligible to enroll yourself and eligible dependents in a health insurance plan. EYA may offer a Health Maintenance Organization (HMO), Preferred Provider Plan (PPO), or a Point of Service (POS) plan. Depending on your geographic location, you may have up to two of these options from which to choose. EYA pays a significant portion of premium costs for your health plan. If you elect dependent health coverage, EYA also pays a portion of that premium. All employees are required to maintain health insurance coverage for themselves. If health insurance is not elected through EYA, proof of coverage must be provided.

Dental Insurance
Dental protection is an important part of a complete health benefits program. EYA provides all employees the opportunity to elect dental coverage through EYA's own Indemnity Dental Plan. Employees residing in Florida have a second option to elect dental insurance through a Prepaid Dental Plan.

Life Insurance and AD&D Insurance
EYA provides, at no cost to you, one times your base annual earnings in life insurance, and one times your base annual earnings in Accidental Death and Dismemberment Insurance. You will be requested to provide beneficiary information for both of these benefits.
In addition to the core benefits provided by EYA above, you have the option to purchase supplemental life insurance for yourself and/or dependent coverage for your spouse and/or children. Additional details on this benefit can be found in the Summary Plan Description.

Disability Insurance
BeneFlex provides short-term disability insurance to all employees at no cost. As part of mandatory benefits, employees purchase long-term coverage through after-tax payroll deductions.
Short-term disability paid by EYA provides 60% of your base weekly earnings to a maximum of $1,900 a week. This benefit will continue for up to 13 weeks after you are out of work for a period of 1 day for an accident and 8 days for an illness.
Long-term disability is paid by the employee with after-tax dollars and is considered a mandatory benefit. This benefit ensures that the employee will receive up to 60% of his/her base earnings to a maximum of $8,000 a month after 90 days of continuous disability.

Business Travel Insurance
Many employees are required to travel on behalf of EYA and as a supplement to basic life insurance, Business Travel Insurance is provided by EYA at no cost for all employees. Business Travel Insurance provides up to $100,000 in coverage if accidentally killed while traveling on company business and $1,000,000 maximum cumulative benefit per accident.

Cancer Coverage
AFLAC offers Personal Cancer Expense Protection to you and your family. Cancer coverage benefits can be used to help cover expenses related to the treatment and recovery of a person diagnosed with cancer. If elected, when you or a member of your family, are diagnosed with cancer, the plan will pay $1,500 directly to you, unless assigned, regardless of any other insurance you may have. This benefit is offered at all locations and paid for by the employee with pretax dollars.

ARAG offers legal protections to you and your family. Legal coverage benefits can be used to obtain the following free services:

Flexible Spending Accounts (FSAs)
The Flexible Spending Accounts (FSAs) allow you to set aside money through payroll deductions that can be used to reimburse you for qualified dependent care and health care expenses. Your eligible health care and/or dependent care expenses must equal or exceed the amount in your account(s) and be incurred during the calendar year. Additionally, you must request your reimbursement or you will forfeit the money in your account. Current regulations prevent your FSA deferrals from being reimbursed to you as regular pay.
- Health Care Account - This account provides a way to reimburse you, and therefore pay, on a pretax basis, most medical, dental, and vision care expenses not covered under other BeneFlex options. You may also cover dependent health care expenses through the account, even if you choose single medical or dental coverage. You may contribute an annual maximum of $3,000 into this account.
- Dependent Care Account - This account provides a way to reimburse you, and therefore pay, on a pretax basis, dependent care expenses. This program allows reimbursement with pretax dollars for work-related dependent day (or night) care costs. You may redirect up to a maximum of $5,000 annually into this account.

Paid Time Off (PTO)
The Paid Time Off (PTO) program is designed primarily for your rest and relaxation. This program allows you to manage your own time away from work, whether it is used for sick, personal, holiday, or vacation time. Full-time employees begin to accrue PTO as of their date of hire. After 90 days of employment, your PTO balance is reflected on your biweekly paycheck stub and available to use. Full-time regular employees accrue 23 days per year. Employees may bank up to 60 days for future use. Employees may also redeem a portion of their PTO balances for cash.

Employee Assistance Program
Eckerd Youth Alternatives offers all employees access to Ceridian's LifeWorks® program. This is an independent, confidential counseling and information resource available 24 hours a day 7 days a week to assist employees and their dependents in dealing with life's challenges.
LifeWorks® can assist with counseling on issues pertaining to emotional problems, marital problems, substance abuse, troubled personal relationships and family difficulties. Additionally, this service can provide information and referral services relating to child care, finances, education, legal issues, and consumer problems.

Direct Deposit
Eckerd Youth Alternatives offers all employees the convenience of direct deposit. Employees are encouraged to have their biweekly paycheck direct deposited to the bank of their choice. Direct deposit is a safe, hassle free way to get checks deposited and avoid standing in line at the bank!

Pension
Employees, after one year of service of at least 500 hours, are enrolled in the Eckerd Pension Plan. EYA contributes 5% of your quarterly earnings each calendar quarter.
